The assessment is educational, not a diagnosis
The symptom assessment, the Root Tracker, and everything else on this site are educational tools. They are built to help you organize a symptom pattern and think about where it might be worth investigating. They do not diagnose any condition, and a result on this site is not a medical finding.
No doctor and patient relationship
Using this site, taking the assessment, or submitting your contact information does not create a doctor and patient relationship with Adam Seay, D.C., or with Root Health NC. That relationship begins only once you have scheduled and completed an actual visit with the practice.
No guarantee of results
Nothing on this site is a promise or a guarantee of any particular health outcome. Every person's situation is different, and past results described anywhere on this site, if any, do not predict what will happen for you.
See a qualified provider. If you have a medical concern, especially an urgent one, talk to your physician or another qualified health provider, or seek emergency care. Do not delay care because of anything you read here.
